DHANYAMLA DHARA
3.1 Definition: It
is a type of Kayaseka, included
under Drava Sweda where Dhanyamla
is poured all over the body in a specific manner for a stipulated time. It is used in Vatarogas, especially with Kaphanubandha

3.2 Materials required:
Dhanyamla- 3 litres
Vessels-
Soft
towels- 3
Oil
for Talam- 10 ml
Oil
for Abhyanga- 100 ml
Rasnadi Choorna- q.s

3.3 Purvakarma/ Pre operative
procedure:
Patient
should sit on the Droni. Talam should be kept on head and Sarvanga Abhyanga should be
done.
Sometimes
Abhyanga is not done according
to the condition of the patient.
Gauze
should be tied around the head above the eyebrows.
Ears
should be plugged with gauze.
3.4 Pradhanakarma/ Procedure:
Warm
Dhanyamla is poured with kernels/mugs by two attendants
standing on either sides of the Droni.
The
temperature should be around 40°C.
Dhara should be poured at a medium speed and
from a height of 6-12 cm.
This
is to be done in the seven positions mentioned in Kayaseka or in Abhyanga
Can
be done hot or cold according to condition. New Dhanyamla should be taken each day.
3.5 Pashchatakarma/ Post operative procedure:
After Dhara, body & head should be
cleaned using the soft towel.
Ear
plugs and gauze are removed &Rasnadi
Choorna is applied to the head.
Rest
is advised for 1 hour and then asked to take bath.
Duration: 45minutes
X 7 days. Preferably done in morning hours in moderate climate
3.6 Precautionary measures:
Dhara should be done in
synchronized manner.
Body should be cover with thick cotton cloth to
avoid shivering.
Fresh
Dhanyamla must be taken to avoid fermentation.
3.7
Complications & management:
Chills & rigors- it usually occurs due
to uneven maintenance of the temperature of Dhanyamla or prolonged time gap between the changing up of fresh
warm Dhanyamla or if body is
exposed to cold breeze immediately after the procedure.
In
this condition, body is covered with thick cloth or warm liquid for drinking or
hot fomentation should be given.
Disinfection and Sterilization-
Before Dhanyamla Dhara
and Abhyanga, Droni should be cleaned
thourolly with the help of detergent and luke warm water. In 5 liter of
water 20 gm of detergent should be added and then Droni should be wiped with
the help of duster. Then again it should be wiped out with the help of dry
duster.
After that Savlon or
Dettol should be sprinkled on Droni, for 5 minutes and then should be wiped
with the help of dry duster.
Other instruments used
for the procedure should be washed thourolly
with the help of detergent and water.
Occupational Safety-
Therapist must wear autoclaved Cotton cap and mask .
They must wear plastic
hand gloves at the time of procedure till the finish of procedure.
Biomedical
waste management-
After completion of procedure
the Dhanyamla should be collected in a vessel and Sodium Hypochloride Solution (in
proportion of 50 ml in 1000 ml/gm) should be added in it.
Let it as it is for 5
minute and then this mixture should be thrown in toilet.
The oil used for Talam
and for Abhyang should be removed and given to patient. If patient refuses to
take this oil then it should be poured in a common can available in IPD, Which
will be transferred by hospital authorities for machineries as a lubrication purpose.
The gloves used by
therapist should be thrown in red
colored plastic bag.
Ear plugs and gauze used
for patient should be removed and thrown in yellow colored plastic bag.
